Revisiting this thread, currently stumped. So, after following your advice, it failing to load, then re-installing the game, I encountered the same issues. I actually get a similar message when trying to open the game through Steam or utilizing the shortcut, except it specifies the .exe for Microvolts instead of the other file.
Once I open it through the .exe, with administrator privileges on both Steam and the .exe itself, I receive the -exact- same error code as listed at the start of the thread.
Maybe this is a permissions issue / communication with Windows Firewall? I'll run some tests.
(Small Edit: The file was the same, one string of code was different. 0023:7B7CB0E9)

Received this exception in the bug report: Microvolts.exe caused ACCESS_VIOLATION in module "C:\Program Files (x86)\Steam\steamapps\common\Microvolts\bin\d3dx9_42.dll" at 0023:77A4B0E9, D3DXDebugMute()+7345 byte(s)
This was after troubleshooting through restarts, verifying caches, running steam & MV as admin. Won't pull up steam log-in either, the one time it loaded stable. Any pointers? I'll be able to tackle this problem next weekend.

Just curious, putting the feelers out. Long time player, joined during OG OBT during 2011, what's the state of the game?
Last time I had played MVS, they had patched out the original float-y nature of the game, making wavestepping very difficult except off of elevated surfaces, and ofc, God weapons / Capsules overpowered the meta 5:1 (of which, I am also guilty in partaking). Is swapping still intact?
Where are we at? In comparison to that 9 yr old opinion of MV.
